Visuals: The sequence begins on a royal blue background with a VHS distortion effect, briefly changing to black and white as the tape rewinds. In the middle of this process, the "PLAY" symbol, alongside text reading "PM 1:40" and "OCT. 26 1985" appear on the left side of the screen for a short period. After more rewinding, a black and white sketch scrolls up, which consists of a monster containing a human head with a screaming expression and melting eyes, and four winds with copies of the head within them. Scrolling alongside it, there is "616 GAMES", which changes to purple when stopped. At random times, either the sketch, text, or both scroll downwards in a sequential order. At the end of the sequence, the logo fades to black.

Variant: The quality of the tape may vary. For instance, on Indicted, there is a large amount of dust in the background.

Technique: 2D computer animation.

Audio: The sounds of record scratching, a VHS rewinding, and a reversed, high pitched voice. Following this, an 80's themed, new age synthesized horror soundtrack plays. In the end, the sound of a tape being ejected from a VHS player is heard.

Availability: The logo is seen on games developed and published by the company beginning with Indicted.
