The timeframe of the flashing images varies with 0:02, 0:03, 0:04, or 0:08

Visuals: On a blue background with a light blue center, there is a yellow circle. In the circle is a green duck with an orange bill who looks a bit like Plucky Duck from Tiny Toon Adventures. The yellow text "SICK DUCK" and "PRODUCTIONS" appear above and below the circle. The duck rolls his head around in a dizzy state before suddenly throwing up. The vomit flashes colors as the duck makes a pretty disturbing face while his eyes pop out and the background begins to move. Then he goes back to normal.

Variants:

On the season 3 The Eric Andre Show episode "Bird Up!", after the duck vomits, a clip of a woman covering her baby's face (a clip from the episode) is shown in the circle.
On the season 6 The Eric Andre Show episode "The Cold Episode", the text above is replaced with "DUCK CAUGHT A COLD", the logo now takes place on a blue background with a white center, and the background doesn't change as the duck vomits. The duck and the "PRODUCTIONS" text appear to be poorly masked, as most of the duck's vomit isn't visible and you can still see parts from the original logo.

Technique: Cartoon-style digital animation.

Audio: There are several different variants:

2012: The duck lets out a spoken "quack!" (most likely provided by André himself) as he throws up and a wobbling sub bass.
2013: A different wobbling sub bass and what sounds like a boar grunting as the duck vomits. Only seen on early season 2 episodes of The Eric Andre Show.
2013-2014: A 3-note organ theme followed by a deep "bakawk" sound. This is actually a snippet of the sound that plays in every "Bird Up" skit, Pop Track N6 by Jeff Newmann.
2018: A stock duck quacking sound effect with water splashing and a stomach growling, followed by a retching sound. Only seen on Mostly 4 Millennials.
2023-: The 3-note theme from the third variant, followed by a stock belch sound effect.

Audio Variants:

On the final season 2 episode of The Eric Andre Show, the "bakawk" is replaced with Hannibal Buress saying "Whack." (which he did at the end of the episode).
On The Eric Andre Show from Season 3 onward (starting with "Naturi Naughton; Ryan Phillippe"), it's a quote from the preceding episode. During Season 6, the 3-note theme either precedes the quote or is played alongside it.
On Eric Andre: Legalize Everything, it's a combination of both main audio variants. The 3-note theme plays first, then the "quack".
