1st Logo (September 16, 2009-May 12, 2013)

Visuals: There is a slide of a young Mike Kelley standing outside a house with a white station wagon nearby. The text "PAGE FRIGHT" is superimposed over the photo. The slide then changes to another image: the white text "a mike kelley production" against a blue background.

Technique: 2D computer animation and a live-action photograph.

Audio: A clicking sound when the slide changes.

Availability: Seen on The Beautiful Life and the first two seasons of Revenge.

2nd Logo (May 24, 2019-August 24, 2021)

Visuals: On a white background is a cartoon drawing of two children standing behind a barrier, looking at large gray canvas with a typewriter on it. The text "PAGE FRIGHT" with a glitch-like effect is seen above. The typewriter types out "BOO" on the paper, causing the children to get scared.

Technique: Cartoon animation, with the typewriter being a live-action image.

Audio: Silence, followed by a typing sound and the children shrieking.

Availability: Seen on What/If and David Makes Man.
