MISC., COLOR, and B&W.
Beside the peephole, the labels "AIR DATE" and "TAPE NO." are margined to each side of the slate, leaving a space above their blanks. An additional blank below the "TITLE" label extends across the slate. The "REC. DATE" and "AGENCY" labels share a row, as well as "COLOR" and "B&W" on the "MISC." blank. An oval "HK" emblem is on the bottom right. The labels, blanks and emblem are in red, except the "COLOR" label (which consists of an order of green, red, yellow, red, blue on each letter). In the peephole, a beige-white numeral in the form of a rear-projector display counts down from 11 to 3.
Variants:
- As WOR-TV carried mostly color programming in 1967, yellow tape covered the "B&W" label most of the time.
- Yellow tape sometimes replaced the "AGENCY" or "MISC." labels with "CUT" written in marker.

Audio: A sawtooth buzzer countdown set between 145 to 170 Hz.
Audio Variant: On a 1967 episode of The Bitter End, a Stage Manager reads the slate.
